Penile Prosthesis Removal and Replacement
A penile prosthesis is a medical device that is surgically implanted in patients with organic erectile dysfunction of various origins that cannot be treated with medication.
Prostheses can also be used to correct congenital or acquired abnormalities of the penis, such as curvature or Peyronie’s disease.
The implant does not alter the turgidity of the glans, penile sensitivity or orgasm, which therefore remain unchanged by the surgical procedure.
When is a penile prosthesis indicated?
In cases where erectile dysfunction does not respond to oral therapy or where oral therapy is contraindicated or unsatisfactory; in cases where intracavernous pharmacoprosthesis (intracavernous injections with prostaglandins) is not effective or is not easy for the patient to perform;
In cases of penile malformations such as penile curvature due to Peyronie’s disease or Induratio Penis Plastica associated with erectile dysfunction (the implantation of a penile prosthesis allows
with a single procedure to straighten the penis, ensuring sufficient rigidity for penetration and satisfactory sexual intercourse);
In cases of widespread fibrosis of the corpora cavernosa as a result of infections, priapism, prolonged use of intracavernous injections;
After destructive surgery in the pelvic area, such as radical prostatectomy, radical cystectomy, colorectal surgery, or abdominal aortic aneurysm surgery, in which erectile dysfunction is an inevitable consequence and almost always unresponsive to oral or intracavernous medication;
After radiotherapy for prostate cancer or other pelvic diseases;
In patients with spinal cord injuries that do not respond to oral therapy, or in whom therapy with intracavernous injections or vacuum devices is uncomfortable.
The main complications are prosthesis infections and mechanical failures. The removal and reimplantation of a penile prosthesis requires a great deal of experience and cannot be performed by inexperienced hands.
